2. Print this form.



3. Fill out the printed form.
   
   For ShoeParlor.com orders your order number begins with the letters "RC". It can be found in your email confirmation or Sales Order PO number. For Special Order shipments sent to you directly by the manufacturer, you may use the PO number on the packaging list. (Usually follows this format: 1-xxxx)
   
   For Amazon.com orders your order number follows this format xxx-xxxxxxx-xxxxxxx. It should be on the packing list included with the shipment you received or in your Amazon order confirmation.



4. Enclose the filled out form in the package that you want to return to us.
