   For ShoeParlor.com orders your order number begins with the letters "RC". It can be found in your email confirmation or Sales Order PO#. For Special Order shipments sent to you directly by the manufacturer: please use the PO# on the packaging list.
   
   For Amazon.com orders your order# follows this format xxx-xxxxxxx-xxxxxxx. It should be on the packing list included with the shipment you received or in your Amazon order confirmation.


4. Enclose the filled out form in the package that you want to return to us.
